The Potential of the New Drug Delivery System D’Liv®

Thank you for visiting the website of Jeisys Medical Japan. Transdermal drug delivery is a method of injecting drugs to the skin with little risk. Numerous drug delivery machines are marketed these days, but some are painful to be treated and some are insufficient to introduce the drug into the skin. Jeisys has introduced a new option for drug delivery, D’Liv®, which offers two modes of delivery to enable drug delivery of Exosome and tranexamic acid with minimal risk. In this issue, Dr. Rieko Horiuchi of Ginza Skin Clinic will introduce drug delivery using D’Liv®.

Skin and conventional transdermal drug delivery (0:25~)

The keratin of the skin has a barrier function, and drugs with molecules larger than 500 daltons cannot pass through the skin. Therefore, various treatment methods have been developed for transdermal drug delivery over the years. An example is penetrating the drug into the skin through the tip of a needle or microneedle. The drug is delivered subcutaneously or intradermally through numerous holes. Mesotherapy,water light injections, and laser treatments well-known in cosmetic surgery are also typical methods of drug delivery. Treatments using needles and mesotherapy are highly invasive and generally require anesthesia. Not all patients are suited for these methods due to the risk of pain, redness, and bleeding. Meanwhile, there are also many transdermal drug delivery treatments with less downtime. Ion delivery, ultrasound, and exporation(electroporation?) are typical treatments. These are less invasive, but the drugs are not always well introduced.

New D’Liv® Drug Delivery (9:20~)

Jeisys’ D’Liv® has attracted attention as a new drug delivery tool. POTENZA, one of the leading micro needling machines Jeisys has worked on, has become popular as an RF (radiofrequency) device with an injection tip. Therefore, D’Liv® was created by improving POTENZA’s pumping tip and excluding RF. D’Liv® is equipped with two delivery modes: pumping mode and injector mode.

Pumping mode (11:01~)

Pumping mode is an introduction mode that uses a 0.35 mm needle. It is equipped with two types of needles: a 16-pin suitable for small areas and a 25-pin suitable for large areas. The needle is inserted while lifting the skin, and the drug is efficiently introduced when the needle is removed. Because the procedure is performed under pressure, it is less painful than standard micro needling. In pumping mode, the needle depth can be finely set from 0.25 to 2.5 mm. Adjustable residual fluid volume, drops, and pumping speed. Suitable for nonviscous liquid, very low invasiveness, and can be used without anesthesia.

【Case 1】13:55~

This patient is concerned about melasma. Little experience with cosmetic procedures. 4 treatments every 2 weeks, 2 full face passes were performed with 0.5mm depth for melasma and 1mm for the whole cheeks. Exosome is selected. The clarity of the entire face was increased and the effect on melasma observed. He (She) was pleased with the smooth skin texture after the treatment. He( She) also felt almost no pain, which he (she)had been worried about.

【Case 2】15:55~

This woman has chronic redness with suspected rosacea from her temple to her entire cheek. She also was troubled by acne scarring due to acne. Tranexamic acid and vitamin C induction was given 4 times, every 2 weeks. Some redness was seen up to the 2nd session, but lightened after the 3rd session. She felt her skin texture improved after the treatment.

This patient underwent Exosome hair growth treatment using the pumping mode; after the third treatment, there was an increase in the bounce and gloss of the hair and a change in the hairline’s rise and density. D’Liv® has the potential to reduce the pain that is often felt with traditional drug-delivery hair growth treatments.

Injector Mode [Water Light Injection](22:03~)

The injector mode, another delivery mode of D’Liv®, is an improved so-called water-light injection. The needle is available in 2 depths, 0.8 mm and 1.0 mm, enabling delivery deeper than in the pumping mode in both cases. Operation is easier than with the mesotherapy gun, and the handpiece is made lighter and easier to use. The settings can be easily changed by operating the screen on the unit. The most distinctive feature of the injector mode is the “viscosity” setting. By adjusting this setting, highly viscous drugs that are difficult to deliver in pumping mode can be injected. Note that when injecting the drug in the injector mode into an uneven skin area, more unevenness is likely to arise than in the pumping mode. There were cases where needles entered too deeply or floated off due to the force applied during injection. Areas prone to drug leakage should be closed with plastic wrap after treatment.

Examples of Induction Drugs(24:57~)

The pumping mode is suitable for introducing Exosomes and other small molecular agents. Whereas, when using hyaluronic acid, the injector mode is more compatible. As D’Liv® is a new machine at this time, the optimal parameters for the drug are undetermined. We intend to establish new parameters through further verification.

Impressions from patients and healthcare providers (26:07~)

Some patients were concerned about redness after the treatment. Others, on the other hand, were positive saying treatment was almost painless, making it easy to continue with it. Some said that their skin felt soft and smooth right after the treatment. Medical professionals have commented that it is easy to suggest to patients as there is almost no downtime. The treatment can be performed nearly without anesthesia, so the process can be performed smoothly with little waiting time. D’Liv® is an easy procedure to perform and would benefit from reducing the burden of learning the technique for staff with limited time.

While a variety of transdermal drug delivery treatments have been developed over the years, D’Liv® is a new drug delivery tool that can be utilized. We expect that the new D’Liv® treatment will reduce patients’ concerns about pain, while at the same time reducing the burden on medical staff.
