Are you considering the introduction of POTENZA at your clinic?
POTENZA is a popular treatment for those who suffer from acne scars and sagging skin with aging.
However, conventional treatment with POTENZA involves the injection of a solution into the skin through a needle, which is invasive.
“I want to be beautiful, but I also hate pain. “
Not a few people will think so.
In response to their needs, Jeisys released POTENZA in 2021 with a non-invasive, non-needle tip.
In this issue, Dr. Lee Ji-min, Director of Kangdong Kodok Clinic of Doctors Dermatology in Korea, a leading cosmetic country, reports on the features and indications of the “DIA tip”.

What is skin aging (00:40-)

It is not clear why skin aging occurs.
However, there are two factors that are probably responsible for aging.

Intrinsic factors of aging phenomenon (03:26-)

  1. Decline in metabolic function due to cellular senescence
  2. Accumulation of ROS
  3. Dry skin due to a decrease in female hormones

As the skin ages, collagen production slows, causing wrinkles and sagging.

External factors of aging phenomenon (05:02-)

  1. UV radiation
  2. Tobacco smoking
  3. Pollution

Ultraviolet radiation, tobacco and air pollution can also inhibit fibroblast activation.
Collagen production is slowed and elastin is reduced, resulting in loss of elasticity.

In addition, the skin’s turnover cycle is halted and aged cells are accumulated.
In other words, aging is due to the decrease in fibroblasts and the accumulation of aged cells.
There are several methods to inhibit fibroblast senescence and stimulate collagen production, such as collagen boosters and RF.

The anti-aging effects expected from RF (8:27-)

RF (Radio Frequency) is high-frequency waves. Simply put, POTENZA is a treatment method that creates electrodes on the skin and applies heat to the inside of the skin by passing radiofrequency waves through them.
Effects can be immediate or long-term.
The first immediate effect that can be expected is a lifting effect.
In the longer term, thermal wound healing stimulates the proliferation of new collagen and the formation of new blood vessels.
This allows new elastin to be produced.
As a result, the skin can be tightened, and skin texture can be improved and moisturized.

Three types of painless handpieces (12:07-)

POTENZA can address and improve various skin problems such as acne scar treatment, rosacea, melasma, wrinkles and sagging skin with a single unit.
By replacing several types of handpieces, customized solutions can be provided for each patient’s individual needs.
In particular, the handpieces introduced at this time are quite unique.
Usually, treatment with POTENZA involves inserting microscopic needles, called microneedles, into the skin.
Although the needles are minute, the pain felt varies from person to person.
However, the three handpieces presented here do not have the “needle” that is POTENZA’s distinctive feature.
This will be accessible to those who are hesitant to undergo anti-aging treatments for fearing pain.

Let me explain one by one.

SFA Tip|Treatment for pores and acne scars (12:26-)

It may be more accurate to describe this as a weak fractional laser.
It uses a 2 MHz bipolar and is characterized by shallow penetration into the epidermis and upper dermis.
It is mainly used to improve skin surface.
Compared to DDR and DIA tips, electrodes are smaller in diameter and more numerous.
The greatest feature is that it can be adjusted to produce single, double, or triple pulses in a single shot.
It has a high degree of freedom of operation, enabling to halve the irradiated area.
With HIFU, the mid-face and around the eyes are avoided to avoid nerve damage, but POTENZA with SFA can be applied to the wrinkles under the eyes and around the eyes.

Risks and disadvantages

A slight redness on the facial surface will occur, but in most cases will disappear within 24 hours. Some patients may develop fine blisters that may turn into scabs. It is better not to repeat irradiation too many times during a single treatment.

DDR tip|To tighten the entire face (14:33-)

It is characterized by the ability to apply heat deeper into the skin than other handpieces. 500 to 1000 shots are applied at a time, without too much pain.
However, it is not very effective at lifting when used alone; DDR is a hand tip that is often used specifically to heat up the entire facial skin.

There are cases of high lifting effect when combination treatment with HIFU is performed after heating up the skin.
HIFU treatments in combination with Thermacool or Oligio may be used, but POTENZA with DDR tips may also be used as an alternative.

DIA Tip|Monopolar and bipolar with alternating irradiation (16:53-)

The electrode size is the same as DDR, but with fewer electrodes.
DIA Tip is characterized by sequential alternating monopolar and bipolar irradiation.
Studies by Korean doctors reported that when a combination of monopolar and bipolar irradiation was used alternately, heat could be transmitted to the dermal layer.
By combining monopolar and bipolar, fibroblasts become active and collagen generation is stimulated.
The pain is only slightly more intense than others with little or no downtime. It can, of course, be treated with an anesthetic cream in advance.
Another advantage is that the effects can be felt immediately after the treatment. But, the effect does not last, so it requires continued treatment every three weeks.

Clinical applications of DIA Lifting (21:57-)

Each person’s face is uniquely different.
Skin is also different for each individual.
We must use various medical devices appropriately according to individual needs.
Just as people change their makeup to match the image they want to achieve, we must choose machines that can target the shallow as well as the deep layers of the skin in order to effectively improve wrinkles, pores, and skin texture.
POTENZA can address both the deeper layer of the skin and the skins surface. In addition, three types of non-invasive tips can be used to meet more specific needs.

Conclusion (28:43-)

By applying heat to reach the dermal layer, POTENZA can be expected to have a lifting effect through collagen contraction.
It is also expected to have a longer-term effect in that fibroblasts stimulate the production of collagen and elastin, supporting the improvement of wrinkles and sagging skin.
