In Japan, people are becoming more beauty-conscious, and many are undergoing cosmetic procedures at cosmetic dermatology departments and clinics. The number of cosmetic dermatology departments and clinics is increasing every year, and many of them are using POTENZA. POTENZA has earned a reputation as an excellent treatment for acne scars.
In this issue, Dr. Hideaki Sato of the Clinica BellaForma talks about the advantages of POTENZA, along with cases from his own clinic. Please watch the webinar by Dr.Sato.

Different types of acne scars and its treatment methods(00:26)

There are 4 types of acne scars.

  • Recessed scar types
    – L icepick types
    – Box car types
    – Round types
  • Hypertrophic scar types

The following methods are commonly used for treatment;

  • Treatment using fillers to make the recessed area swell up.
  • Treatment using a laser or a CO2 laser to ablate the raised area
  • Chemical peeling(TCA)

Triple synergy effect of acne scar treatment with POTENZA(02:30)

  1. Wound healing effect by puncturing ultra-fine needles
  2. Coagulation and tissue regeneration by monopolar RF irradiated from the tip of a needle
  3. Tissue volume augmentation by PLLA drug delivery

POTENZA is a treatment method in which ultra-fine needles (0.25 mm) are inserted into the subcutaneous area. By utilizing the wound healing process, the body’s mechanism for repairing skin wounds, it is expected to lead uneven skin to smooth and fair skin. Also, by irradiating RF from the tip, heat energy is applied directly to the dermal layer to stimulate the production of collagen and elastin. POTENZA’s drug delivery system with a unique pumping tip allows for precise penetration of medication to the target, resulting in tissue volume augmentation.

What is McCoom?(03:00)

McCoom is(It is) a PLLA preparation with fine particles developed exclusively for Potenza. It is less likely to cause allergic reactions and has already been used for many years in the medical field. It stimulates the production of collagen and elastin and is expected to have efficacy in the treatment of acne scars and uneven skin. It takes several months to around two years to decompose into water and carbon dioxide.
Around 6 months after treatment, the inflammatory response improves and type I collagen increases significantly. Recent studies also suggest the presence of type III collagen.

Protocols(04:41~)

We use POTENZA’s pumping tip PC21.
Because of coagulation by RF, we don’t see any significant bleeding.
We perform stacking and RF with finely tuned level settings.
At first, stacking is performed at level 10, then the level will be lowered to level 1 or 2 to inject dissolved McCoom.

This is a side view of the pumping tip.It contains 12 microneedles with a tip of 0.25 mm. The penetration depth of the needle can be adjusted up to 2.5 mm according to the area.

Mechanism of the pumping tip(15:40~)

Air flows in when the needle is inserted, then the skin is pushed up and lifted. This allows the needles to be inserted accurately at an even depth.
Air is pushed out when the needle is removed and the medication flows in instead,The drug delivery effect of using this pumping tip can lead even the most severely uneven acne scar to smooth skin.
The heat energy from RF irradiation inhibits the formation of new capillaries(inhibits the angiogenesis) in the dermal layer, which can be expected to lead to improvement of redness around acne.

Comparison of POTENZA, microneedling and mesotherapy(15:57~)

There are several cosmetic treatments using needles, let’s compare them with POTENZA.

Microneedling

Microneedling treatment can only puncture the skin in a shallow area, so it’s actually like causing just damage to the skin.
Besides, medication is pushed back by the exudate from the punctured area.
Downtime depends on the number and depth of needle insertion.
With microneedling treatment, the downtime was short, but the efficacy was only low.
POTENZA, on the other hand, allows for deeper and more stable needle penetration and reduces bleeding due to the thermal coagulation effect of simultaneous RF irradiation.Downtime can be as short as 2-3 hours if treatment is performed at the same depth as microneedling.The pumping function ensures that medication is delivered properly to the target area.

Mesotherapy

Mesotherapy is a very difficult procedure to control the insertion depth and to deliver medication evenly.
There is also longer downtime due to pain, bleeding, and swelling caused by needle insertion.
Potenza can inject the medication evenly to the precisely same depth, ensuring that the same quality of care is provided at all times.
