Thank you for viewing Jeisys’ website. Recently, aesthetic medicine is no longer only for women. Dr. Hideaki Sato, Director of CLINICA BellaForma, will present cases of sagging skin treatment using ULTRAcel[zi:], which has been requested by many men who are interested in aesthetic treatment.

Male, 45 years old. He wants a lifting of the entire face. HIFU, ULTRAcel[zi:] will be used in this case. To decide where to apply the HIFU and with what intensity and depth, mark the areas of the face. The positions are based on the bones of the face. The area above the bones should not be strongly irradiated.

Mark the mandibular line and masseter muscle. Apply HIFU at 4.5 mm subcutaneously to work most effectively on the SMAS. Care should be taken where the orbital nerve is located. Also, be careful not to irradiate strongly in areas of extreme indentation, such as the temples and forehead.

The forehead is also irradiated at 0.1 mm depth with 2 mm cartridge instead of 3 mm because of its thin subcutaneous tissue. The same applies to the temples.The SMAS area is irradiated in 4.5mm Dot Mode, and the submental area is also irradiated in 4.5mm Linear or 3mm Dot Mode(0.6J).

Start treatment from the submental area, using 3 mm Cartridge with 0.6J. Apply at 0.1 second intervals in Auto Mode, but do not apply directly to the bone line marked earlier.

The cheek is irradiated at 4.5 mm in Dot Mode, 0.8 J. From the masseter muscle to the SMAS, the cartridge is applied sliding sideways to reduce the risk of nerve damage.

It is recommended to change the target depth from 4.5 mm to 3 mm near the corners of the mouth where the nerves are located shallower to avoid nerve damage. 0.6 J irradiation can be expected to reduce fat. The cheeks are irradiated vertically. The temples should be irradiated with 0.1 J because of their hollows.

Around the eyes and forehead are irradiated at 2mm (0.2J) in Dot Mode. Irradiation of 3 or more passes around the eyes is more effective.

The forehead is usually irradiated at 3 mm (0.1 J), but this time about 2 passes were given at 2 mm (0.2 J).

Similarly, mark the location of the facial bones. If you mark the position properly, it is easy to know which depth of the cartridge to use at what intensity of joule. If you also mark the anterior masseter muscle, you will know how far you can do it at 4.5 mm depth.

HIFU at 3mm to the orbital fat is too strong and may reduce the fat too much, resulting in loss of facial definition.Irradiate the hollow areas of the cheeks with care. It is safer to apply the treatment so as not to reduce the fat. This person has a prominent so-called gorgon line, but we can aim to improve it by lifting the entire area.

Apply at 3mm (0.6J); change to 4.5mm (0.8J) to target SMAS. Do not irradiate the hollow areas.

Change to 3mm Dot Mode. The hollow areas are treated with 0.1 to 0.2J. Korean studies have reported that it is possible to increase fat, but it may be difficult to achieve it without a substancial number of applications. It is safer to apply a lower number of joules to avoid further denting.

Under the eyes, change to 2mm (0.2J) Dot Mode. Forehead is also the same. The irradiation is further switched to Linear Mode, which is very painful, so the location should be changed every 0.2 seconds. An intensity of about 0.4 J is recommended.
