Sagging is difficult to improve with home care as it is caused by a loss of collagen and elastin and loosening of the SMAS fascia. Many beauty clinics are therefore using HIFU to improve sagging skin. In this issue, Dr. Hiroko Sugino, Director of Aoyama L Clinic, will introduce the premium HIFU treatment using ULTRAcel[zi:].

Principles of HIFU (00:37~)
HIFU ultrasound is emitted by a round, concave transducer that focuses at a set distance and produces frictional heat in dots on the skin tissue. It also causes the tissue to contract because the transducer moves in line and produces heat in dots.
History of UltraCell (3:31~)
The 1st generation of ULTRAcel was introduced in 2014; 1 unit is equipped with HIFU, monopolar RF, needle RF, and non-needle RF handpieces to transfer energy from the epidermis to the dermis.
In 2018, the ULTRAcelQ+, a HIFU-only device, was introduced. The irradiation speed has been increased as the transducer moves through the probe at high speed. Linear irradiation allows for the delivery of thermal energy over wider area.
The 3rd generation ULTRAcel[zi:], introduced in 2022, allows switching focus depth and mode without changing cartridges. It can efficiently heat the dermis to subcutaneous tissue and SMAS fascia.
Features of UltraCell Zi: (5:44~)
The point irradiation (dot type) of UltraCell Zi performs skin lifting and tightening. Linear irradiation (linear type), meanwhile, delivers heat over a wide area to destroy subcutaneous fat cells and contour them.
The focus depth of any cartridge can be set at 0.5 mm intervals. Since each setting can be switched, only four cartridges are needed for treatment of the periocular area, face, and body.
Dr. Sugino’s Treatment Method (6:59~)
Our HIFU treatment method has evolved along with the evolution of ULTRAcel. Currently, we use ULTRAcel[zi:] to provide a combination of dot and linear treatments.
Treatment with URTRAcel[zi:] is basically provided on 1000 lines, 40% more lines in the past.
First, dot type (4.5mm, 3.0mm, 1.5mm) is used for a total of 500 lines for lifting. Next, linear or dot type (4.5mm, 3.0mm) is used for fat burning and contouring by heat accumulation.
Upper eyelid sagging treatment with HIFU has also been popular, using dots (2.0 mm) to irradiate the frontal area, temples, and upper eyelids.

Precautions for safe treatment (15:50~)
Consumer Affairs warns about the risks of HIFU treatment: HIFU risks burns, paralysis, and numbness, it should be done by a physician familiar with the thermogenic mechanism and the facial anatomy. During the treatment, always be aware of the depth of the irradiation and keep the probe in the correct position.
