What is POTENZA (01:12~)
POTENZA is a medical equipment that heats tissue with various tips and RF (radiofrequency) for a wide variety of cases. Currently, 15 types of tips are available, allowing for a gradual assortment of tips to meet the needs of clinics and patients.
POTENZA’s energy is not melanin-selective, so there is no need to take skin type into account, but the electrical properties of the organism (conductivity, dielectric constant, etc.) should be considered. We are always thinking about what the optimal settings are for our treatments.
Reasons Why Our Clinic Introduced POTENZA (05:52~)

POTENZA’s ability to treat a wide range of skin problems was a reason for considering its introduction.
The other reason was that as the number of cases increased as the number of patients grew, we needed to find a treatment method that could stabilize epidermal melanocytes in addition to the conventional treatment of hyperpigmentation.
In some cases, improvement of skin tone is time consuming or difficult. For patients with high skin type numbers, the primary treatment target is melanin, but we believe that treatment with non-melanin-selective devices is also necessary.
POTENZA is one of many medical devices that utilizes thermal effects. Many papers have been published on the biological effects of thermal effects, and the search for melasma treatment continues. Of particular interest is a paper describing a case in which RF increased HSP70, which is thought to be involved in the regulation of p53, and decreased melanin synthesis.
In melasma lesions, the WIF-1 gene is decreased, increasing melanin formation and presenting symptoms similar to post-inflammatory hyperpigmentation, but H19 and WIF-1 are particularly decreased. It has also been shown that estrogen induces PDZK1 expression and increases MITF and tyrosinase expression. In other words, inhibition of PDZK1 is expected to contribute to the treatment of melasma.
Although melasma is generally difficult to treat, it is expected that optimal RF irradiation can suppress tyrosinase activity.
Examples of Treatment for Pigmentary Hyperplasia and Melasma (17:52~)
An example of treatment for melasma. In melasma lesions, the expression of genes involved in lipid metabolism is decreased and barrier function is impaired. Because it is aggravated by friction, it is necessary to carefully monitor the entire face, not just the lesions.
The expression of genes involved in lipid metabolism (PPARA, ALOX15B, DGAT2L3, etc.) is decreased in melasma lesions, resulting in impairment of barrier function.

After three treatments with the S-16 tip, she herself has noticed a change in skin texture.

This is a case of difficult treatment of melasma. 10 times of RF and tranexamic acid were used. 3 times of irradiation was ineffective, and then the wattage was increased.

She had post-inflammatory hyperpigmentation and melasma. Both skin tone and texture improved with treatment. To maintain this condition, she visits the clinic every 2 months.

Since POTENZA is not a melanin-selective treatment, we feel that its effectiveness is further enhanced when combined with melanin-selective treatments in an appropriate manner.
In this case, after 3 sessions of IPL irradiation, 3 sessions of POTENZA treatment with S-16 tip were performed.
3.Treatment interval: every 4 weeks 5sessions in total
4.Others: After 3 sessions of IPL at one-month intervals, RF was started.
Although Potenza alone is of course effective, we believe that a combination of melanin-selective and non-melanin-selective treatments with different thermal effects is more effective.
